Jeff,

If you secured a judgment in court, you don't have to personally record the judgment. The CLERK'S OFFICE routinely handles the process of recording the judgment. 
On occasion, the Clerk's office might overlook a judgment. If you don't double check and figure out that the judgment didn't get recorded, then the real property that is ordinarily liened can be sold without a satisfaction of the judgment. That doesn't happen that often. 
So, in order for the lien to be satisfied and removed, YOU must file a satisfaction of judgment with the court.

The CLERK'S OFFICE will then record it, or its a matter of public record, not sure which one.
That's how title companies know if a judgment is or is not still outstanding.
