[WSBARP] Affidavit that Unlawful Detainer for "health and safety"

Hi Kaitlyn, I am surprised no one has produced a form for you by now. Perhaps no one has generated one yet.

Sadly, I do not have an affidavit form either. However I will say that in the last 10 days I successfully mediated 3 tenant exit plans for similar scenarios, entirely online. One (in the Bay Area, where the laws are much tougher on landlords generally) was extremely aggravated, but a cash for keys deal negotiated through a professional neutral made the difference. 

Covid-19 is illuminating serious problems in space - sharing arrangements that have been there along, largely ignored by everyone in so far as it is a major driver of housing conflict and evictions. There will be an explosion of such cases in short order, and traditional means of handling them are not up to the task.
