[WSBARP] PRESIDENTIALLY DECLARED DISASTER EXTENSIONS FOR 1031 EXCHNAGES

To 1031 Exchange practioners: The President declared Washington a major disaster area due to Covid-19 Pandemic on 3/22/20.Under Rev. Procedure 2005-27 a tax payer may qualify for extension to 45day identification and 120 day Exchange period deadlines. I would like verification of that and I know that an application for extension has to be made but I can't find a form for that so is it done via a letter and to whom at IRS should it be addressed. Thank you
