Hi Dave, 

Whatever the Identification document that was used when notarized should be used. I’m assuming grantee’s ID is not checked when a deed is to them as grantee. I think title would want to have the deed re-executed and re-recorded later to clear the errors from Bert Smith to Albert Smith. If grantor’s are long gone it might be difficult. I don’t think it is a problem until Title gets involved. I would suggest to title (if there was no way to chase down the original sellers) to have a deed from Bert Smith to Albert Smith to correct the name issue. 

Every title company is different. I have had various disputes with them before and sometimes they are not always right (as confirmed by e.g. DOR on excise tax issues I’ve disputed with title in the past).
