[WSBARP] Affidavit of Identity

This is the language we in the title industry look for.
An affidavit might be useful when the facts are too complicated for inclusion in a deed, but in the facts you gave I think the language Jennifer suggests would work.

Can you do a deed from "X, who acquired title as Y" to whomever he/she is conveying the property?
