I just did this a few weeks ago. Here is a copy of my Petition, along with a copy of the Order that was entered. Your language will obviously differ from mine, but this should give you a good start.
Let me know if you want this in a Word version.

Subject: [WSBAPT] Special Administrator for Will in Safe Deposit Box
Hi list,
Does anyone have sample pleadings for appointment of a Special Administrator to access a safe deposit box under RCW 11.32.010 that you are willing to share? Decedent had no spouse, child, parent, or other family member that any of us know of, and we are virtually certain that the original copy of the Will is in the decedent's safe deposit box. Our local court has no rules granting us access. It looks like special administrator is the only route. I could muddle through drafting these myself, but my mentor always told me "if you aren't committing plagiarism, you are committing malpractice!" so I'm hoping one of you is willing to help out. Thank you!
Best,
David J. Faber
