[WSBAPT] Guardian replaced

Hi, an Elderly woman had guardianship of her children and wanted a backup. An atty referred to her but instead replaced her by representing to the court that she wanted out.  But there was no resignation from the Guardian or any declaration to support this. Is it jurisdictional? Can a court do this without notice, without some sort of signed declaration or signed Petition? Can atty just say I represent so and so and that my client wants out -
