[WSBAPT] Amended and Restated Trust- Deed Question

I have recorded a Notice of Change of Trustee and Restatement/Amendment of
Trust for the benefit of the title companies. The other day I was having a
discussion in the office with an attorney whose client did a restatement and
the drafting attorney goofed up the reference date to the original RLT. They
were considering recording a notice of change of trustee and Restatement to
address the incorrect reference to the date, because the deed vesting title
in the RLT referenced the date of the trust agreement. I think this is a
circumstance where a notice is warranted.

H&W had revocable trust.  H has passed.  Trust remains revocable.  W is
going to perform a complete amendment and restatement and also add son as
co-trustee.  Does anyone prepare a new deed to reflect the change of
trustees and the restatement?
