Second to John’s suggestions about using SWD and verifying that title insurance will carry over. Use of a QCD may, under some title policies, result in loss of coverage.
Also, make sure the fire/property/casualty insurance carrier is advised of transfer to trust so that trust becomes insured under fire policy. I recommend a letter to the agent/carrier to create written notice.

Make sure the Grantee is the client as trustee, adequately identify trust by name, and date of agreement. I usually describe consideration in the deed to be “his acceptance in trust” in case the DOR wonders about the REET. 
 
Think about whether a SWD is more appropriate for title insurance purposes. Make sure homeowner coverage carries over.
