[WSBAPT] Disclaimer Trust Trust???

Over the years, I have prepared many Disclaimer Trust Wills and many
Revocable Trusts.  Now I have a call for use of both concepts with the
same couple.  How does one do that for an elderly couple who own real
property in multiple states (indicating use of a Revocable Trust) worth
about $3 to $4 mil (indicating use of a Disclaimer Trust Will)?  I imagine
that could look like a revocable family trust type of document with the
surviving spouse/trustor being able to disclaim into a separate disclaimer
trust?  Is this correct?  Does anyone have an example or form for such a
beast?   Feel free to respond offline if I have this completely wrong.
