My understanding is that the table is only necessary if the information is not set forth on the first page of the document.  That said, I routinely include the table anyway.  (It goes without saying, but “what’s required” and “what the person processing the request thinks is required” are often two separate questions).

Dear All:

For routine documents to be recorded such as a quitclaim deed or deed of trust, I have always included what I will call a "table summary" on the first page that looks something like this:

Then, in the language that follows, I identify the parties again and either insert the full legal description or refer to an Exhibit A that contains the full legal description.  I would like to think the use of a table summary like the one above helps the recording office index the document more efficiently.

However, if the body of the document on the first page identifies the full names of the parties and contains the full legal description (without reference to an exhibit) and the affected parcel number(s), is there any requirement or need to have such a table summary at all?
