I doesn't look like these questions have been answered in this thread, so here is what I think.

I would list the original grantor as the grantor on the revocation.  The original grantee, who has not received anything (at least yet), does not have anything to grant back to the original grantor.  Also, the revocation could alternately be described as a "negative grant."  In other words, "the grantor gives and the grantor taketh away."

No REETA is required.  See WAC 458-61A-303 (3) (k).

Two more questions... is the grantor the original grantor who made the TODD or do we reverse the parties on the revocation?

Also, how are you dealing with the REETA. Is one required?
