[WSBARP] contract for live-in caregiver

Dear Colleagues,

I have a client recently discharged from a nursing home who has hired a
live-in caregiver who will reside in a basement apartment and be paid
$3,000 per month.

Would anyone kindly share with me a form (probably a license rather than a
lease) which addresses all the things which could wrong in such an
arrangement.
