I think you could just have Nonprofit 1 quitclaim it to Nonprofit 2

Nonprofit 1 owns real property.  Nonprofit 1 then formally changes its name
(and name only) to "Nonprofit 2" by filing with the Secretary of State.
Does anything need to be done, title-wise, to make it clear to everyone in
the world that the real property is owned by Nonprofit 2?  

I think I've been overthinking this one.
