Attached is the health care directive I use.  I’ve been told by several physicians that it is very comprehensive.  Please feel free to use as you wish, and anyone else who wants to use it is welcome to do so as well.

Hello Listmates.

I have a very simple and adequate Directive to Physicians.  However,
clients keep asking for a much more complicated one that gives them choices
and boxes to check and things to identify.  I have seen them.

Can someone send me a Word document of the more complicated Directive to
Physicians?

I really don't want to type it all up from scratch.  A model would =
be most helpful.
