Chances are, it is a dedication of right-of-way form.  Municipal ROW is treated like an easement, but actually it is something a bit different.  You may very well want the dedication form, because it is typically easier to vacate a right-of-way than it is to release an easement.  Moreover, if the City were to vacate the ROW, there would remain the easement.

To everyone who is suggesting the city should draft it and/or ask the city for a form: 

 

They have a  general form and the language is more like the property owner is giving away his property. They call it a "dedication of property to the city." I am not comfortable with it. They have agreed for us to change the language but they don't have any other forms and/or won't redraft. They want us to draft and they will review.
