[WSBAPT] Transfer of Trust Property when Trust revoked but not retitled

I am assisting with a probate.

H&W had a trust got a trust in the late 90's. They actually re-titled
assets. Then they re-did their estate plan and went to a Last Will and
Testament, but a passive income real property was never transferred back
into their names.

Now H has died.  W is in memory care.  POA for W who has been taking care
of both of them for years has no documents other than the most recent
estate plan and didn't even know there had once been a Trust.

I am unclear as to the path for getting the property from the Trust into
W's name.
