[WSBAPT] Which name to use

Jeff,
I don't have a legally supported answer, but I typically use f/k/a or a/k/a when the client presents with a mish mash of various combinations of names that have been used inconsistently on official documents like driver's license, SS, employment records, tax returns, voter registration, titles, deeds, etc.

Listmates:

Client has an revocable trust using her name S.S.B.  Her married name is S.B.W..  Her Will, from a different lawyer than the one creating the trust, has her name as S.B.W.  She deeds her house into trust saying the Grantor is S.S.W to S.B.W, Trustee of the S.S.B. Revocable Trust.  Client's financial advisor now tells her to use, as her name, S.S.B.W.  Do you see a need to add this 4th name to anything?  How often do you use "f.k.a." or "a.k.a." in Wills/Trusts, even POA's with clients that had other names?
