[WSBAPT] help!

Anyone out there have their initial pleadings for nonprobate notice to creditors?  I have a Declaration and Oath, Nonprobate Notice to Creditors but am not liking my initial pleading document that let's the court know that the intent of the procedure I am undertaking is to provide nonprobate notice to creditors.

Any insights/help is greatly appreciated.
