[WSBAPT] TEDRA - hearing scheduling - Snohomish County

How does the Snohomish County Clerk go about scheduling a TEDRA hearing on application of a party?  I don't see a Sno County form for applying for a hearing.  Do you make one up?   And do I have to draft a notice of hearing for the clerk to sign?  or do I just get a date and put it in the notice?   And is it set before a judge or a commissioner?

RCW 11.96A.100 (4): "Subject to other applicable statutes and court rules, the clerk of each of the superior courts shall fix the time for any hearing on a matter on application by a party"
