OMG. Eleanor, bless you for mentioning where the forms are available online. I don't recall anyone ever actually mentioning where they were, and it's not findable via the Clerk's office "forms library"; it's completely separate under the Court's Ex Parte page, and not under the Court Clerk's Forms page. It is really confusing sometimes to have a "Superior Court" webpage and a "Superior Court Clerk" webpage, with different resources on each.

Dalynne,

You can find the addendum for blocked account form on the King County Court website under minor settlement forms. It should be submitted with the proposed order to approve the settlement. In my experience as a settlement GAL, it is usually the petitioning attorney who fills it out and submits it, but in my role I confirm that someone is doing it, especially if the petitioning attorney has not handled a lot of minor settlements. Otherwise the process is held up, as you have experienced.

All, I was referring to the settlement GAL addendum required in all King County settlement GAL cases.  The commissioner reiterated in hearing last week that those are required and the attorney had not provided one for the court to review.
I could not find it online then and have one I had saved - attached.  I thought it might be updated.  Maybe someone can find an updated version online.
