[WSBAPT] 60 day Notice to Sell Affidavit

Hello mates,

I need to issue my first 60 day vacate notice under the new proclamation
that now requires the notice be in the form of an affidavit.

Does anyone have a template affidavit they have successfully used for this
purpose that would be willing to share?
