[WSBAPT] Medical POA - with power for Involuntary Commitment

Dear Brent,

I have used a mental health advance directive for this purpose, and a copy of that form is attached.  I have not needed to use it for several years so you might want to check it against the statute in case there have been any changes, but feel free to use this otherwise as you wish.
