Second the Mental Health Care Advance Directive.  There is a form in the statute.  I also recommend you contact Lisa Brodoff at Seattle U Law School who was instrumental in developing this tool.

I have a client who is looking to expressly grant her Medical POA the authority to involuntarily commit her for mental health reasons. 

 

Has anyone do an advanced POA like that - and willing to share?
