[WSBAPT] FORM Certification of Trust under RCW 11.98.075

Good afternoon:
Does anyone have a form obtained from, or used successfully with, a title company or financial institution for a Certification of Trust as provided under RCW 11.98.075? The statute lists the required contents, but I'm looking for a tried and true form someone has used successfully.
