[WSBAPT] client with MS

I have a couple who wish to update their estate plan to include transfer on death deeds. The husband while mentally competent, he is unable to sign his name. He can however, mark with an X.  I know that this can be done, but was wondering what sort of documentation I should prepare or place in the file in case anyone questions his signature later? Any suggestions on how others have handled this would be appreciated.
