[WSBAPT] Assignment of copyright

I'm handling a probate in which the decedent had several copyrights on published material.  Her assets are passing to a trust and I'd like to assign the copyrights to the trustee.  They aren't especially valuable, but they need to be dealt with.  Does anyone have an assignment form or advice?
