In the context of a distribution to a minor from an estate I have been successful in obtaining an order stating that the minor’s share shall be deposited to an account in an insured financial institution for the minor’s benefit subject to withdrawal only upon the order of the court in the probate proceeding, or upon the minor attaining the age of eighteen years and furnishing proof thereof satisfactory to the depositary. See RCW 11.76.095(1). This was in Clark County.

Sincerely,

I think you will find what you are looking for here: http://www.kingcounty.gov/courts/scforms/minorsettlement.aspx. This is for King County but should give you an idea of what the form should look like. Mary

I have heard commissioners approving deposits to blocked accounts for minors allow withdrawal / disbursement upon submitting proof of attaining age 18; and on other occasions, the commissioners have required (in the process of crafting the order approving deposits to blocked accounts) that the individual file a motion and make a court appearance for an order directing the bank / custodian to release the funds. What is the criteria or your experience in crafting the order for deposit to a minor’s account?
