[WSBAPT] Loved One to Nursing Home - overwhelmed

My mother in law who has been living in Tennessee for the past 10 years is now going to be moving to be placed in a nursing home facility in Washington.  She suffers from dementia as to short term decisions but has a wonderful memory long term.  She gets social security and a small pension (under $26,000 a year total) and has no real assets - home & bank accounts were transferred to step-sister about 5 years ago after a stroke and the death of her husband.

Can someone point me to a resource or someone to talk to about these elder care issues in order to make decisions involving her care, placement, Medicare/Medicaid issues, Kitsap County nursing home facilities and ratings and how to make these decisions?  We are overwhelmed.  Is it 6 years look back to the transfer of assets before she will qualify for payment of long term care?
