Widow has a “Potential Private Retirement Benefit Information”
sheet from the Social Security Administration, mailed to her but with
husband/decedent’s name and SSN referenced. It says, “We are writing to
tell you that you, or the worker whose Social Security number appears at the
top of this form, MAY be entitled to some retirement benefits from a private
employer. Also, your family, or the worker’s family, may be entitled to
retirement or survivor benefits.” It lists the Plan Name, Plan Number, Plan
Administrator and Address, Year Reported (2000), Type of Benefit, Payment
Frequency, and Value of Account.

The decedent’s employer (listed correctly in the Plan Name and
Plan Administrator) was bought out by another company presumably after he
retired. The new company informs us that it cannot find his name, SSN, or
Plan Numbers in its system. It also told us that Great West Financial was
the “provider/plan administer/vendor” for the first company's retirement
funds, and that Great West would continue to hold such funds for anyone who
retired before the company takeover.

Great West Financial informs us that it cannot find the name, SSN,
or Plan Numbers for the decedent in its system.

Is there any way to track possible benefits that anyone knows
about?

          Thanks.

Rob
