It says, “We are writing to tell you that you, or the worker whose Social Security number appears at the top of this form, MAY be entitled to some retirement benefits from a private employer. Also, your family, or the worker’s family, may be entitled to retirement or survivor benefits.” It lists the Plan Name, Plan Number, Plan Administrator and Address, Year Reported (2000), Type of Benefit, Payment Frequency, and Value of Account. <o:p></o:p></span></p><p class=MsoNormal><span style='font-size:14.0pt;font-family:"Times New Roman","serif"'><o:p> </o:p></span></p><p class=MsoNormal><span style='font-size:14.0pt;font-family:"Times New Roman","serif"'> The decedent’s employer (listed correctly in the Plan Name and Plan Administrator) was bought out by another company presumably after he retired. The new company informs us that it cannot find his name, SSN, or Plan Numbers in its system.
