[WSBAPT] Questions re: co-trustee acting with trustor-trustee

Listmates,

 I am drafting a trust for an elderly client who would like to name her
daughter co-trustee to act with her.  I have two questions:


1.	Should the daughter sign the trust document as trustee or should
she sign a separate acceptance of trustee document? 
2.	Since there is one trustee that is not the trustor, does the trust
now need an EIN or does the fact that a trustor is still a trustee negate
that need?
