Dear Parents,
The Idaho North Central District Public Health will be sponsoring H1N1
flu vaccinations for students in grades 7-12 on Monday, November 23,
from 3:30 - 6:30 at the Moscow Junior High School Multi-Purpose Room.
There is no cost for this vaccine. If you have any questions about the
vaccine or the vaccination clinics, please call Public Health at:
1-866-736-6632.
The informational letter from Public Health is enclosed with this email
and the consent form and other important information is available at the
Moscow Junior High School or Moscow High School offices. The complete
consent form and other information is also available at the Moscow
School District website: www.msd281.org <http://www.msd281.org/> .
If you have a child in Moscow Junior or Senior High and want to get this
vaccine, please complete and sign the consent form and bring the consent
form to the Public Health Department's vaccination clinic on Monday,
November 23.
All questions should be directed to the Public Health Department.

Dear Parents/Guardians:
As you may have heard, a new virus, called the 2009 H1N1 influenza
virus, has caused illness in many children, and there have been large
outbreaks in some schools across the country. The Centers for Disease
Control and Prevention recommends that children and young adults aged 6
months through 24 years be vaccinated.
There are two types of H1N1 vaccine for use in these school clinics. One
type is the inactivated injectable "flu shot"; and the other type is
live attenuated "nasal spray vaccine". Please refer to the attached
Vaccine Information Sheets for more information. The type of vaccine
your child will receive will be based on information you provide on the
consent form.
Vaccination is the best way to protect your child from this potentially
serious disease. Public Health - Idaho North Central District (PH-INCD)
is working with your child's school to give the 2009 H1N1 influenza
vaccine to children at school. We will hold vaccination clinics
beginning this fall. When PH-INCD receives the vaccine, clinics will be
scheduled; you will be informed of the dates and times. Children less
than 10 years old will need two doses of vaccine spaced about four weeks
apart. There will be no cost to you for this vaccine.
To Accept (Consent):
Read the "2009 INFLUENZA VACCINE WHAT YOU NEED TO KNOW FORM" included
with this letter. If you have a child in Moscow Junior or Senior High
complete and sign the attached consent form and bring the consent form
to the Public Health Department's vaccination clinic on:

To Refuse (Do Not Consent):
Do not return the form.
