influenza virus, has caused illness in many children, and there have been large
outbreaks in some schools across the country. The Centers for Disease Control
and Prevention recommends that children and young adults aged 6 months through
24 years be vaccinated. <o:p></o:p></span></font></p>

clinics. One type is the inactivated injectable “flu shot”; and the
other type is live attenuated “nasal spray vaccine”. Please refer
to the attached Vaccine Information Sheets for more information. The type of
vaccine your child will receive will be based on information you provide on the
consent form. <o:p></o:p></span></font></p>

fall. When PH-INCD receives the vaccine, clinics will be scheduled; you will
be informed of the dates and times. Children less than 10 years old will need
two doses of vaccine spaced about four weeks apart. There will be no cost to
