I have a very simple and adequate Directive to Physicians. However,<br>
clients keep asking for a much more complicated one that gives them choices<br>
and boxes to check and things to identify. I have seen them.<br>

Can someone send me a Word document of the more complicated Directive to<br>
Physicians?<br>
<br>
I really don't want to type it all up from scratch. A model would =<br>
be most helpful.<br>
