[WSBARP] Template for Mtn for Surplus Funds

Hey Listees, house gets foreclosed.  Trustee's surplus funds are deposited with local superior court.  My client is entitled to the surplus.  Anyone have a vanilla form/template to share for a motion requesting disbursement of the surplus funds and proposed order?  Thanks.
