[WSBARP] Off Topic Question - Interrogatory regarding Party's criminal history

This is one of the standard Interrogatories I use in my PI cases:  "Identify the case name, case number, and nature of any criminal or civil case or lawsuit of which you have been a party within the past ten years."

As may already know, you can also look up Washington Superior cases (criminal and civil) in the Oddysey portal: https://odysseyportal.courts.wa.gov/odyportal

I usually do not ask for a party's criminal history; so my interrogatory forms lack such a question.  Now that issue is relevant in a current case.  I have seen detailed interrogatories on criminal history, which I am sorry I did not keep.  Would any of you have such an interrogatory that you could share with me?
