[WSBARP] Shared Water Service HOA for Short Subdivision

Hi everyone:

A builder client is finishing four-unit subdivision with a single, shared
water system running through one fire water service and one domestic water
service, i.e. one water service for all four units.  Prior to connecting
the water service, Seattle Public Utilities wants an HOA or other entity
established to maintain the shared service, and to pay SPU.

Has anyone dealt with this issue before?  Is there an alternative to
creating an HOA?  Or creating a very simple HOA?  I'm familiar with most of
the other requirements, but not the creation of the HOA.  Any sample forms
or advice is greatly appreciated.
