[WSBARP] Foreclosure Gurus - Surplus Funds

Hey and belated happy 4th!!  Held a foreclosure sale the other day and there were bids above the foreclosing lender's credit bid.  Anyone have the form where I can deposit the surplus funds to the court under RCW 61.24.080?  None of my sources, including WA Lawyers Practice Manual seem to have the form.
