[WSBARP] Contract forfeiture

Subject: [WSBARP] Contract forfeiture

 

I need to conduct a forfeiture on a very small contract.  Would anyone be
willing to share the current forms for the Notice of Intent to Forfeit and
for the Declaration of Forfeiture?  Mine are sadly out of date..

 

Or maybe point me to a source for the documents?

 

I appreciate any help.
