[WSBARP] Notice re Manganese (or arsenic)

First I’ve not heard of a specific disclosure for arsenic in Island County.  I don’t think there are any Island County ordinances to that effect.  Maybe have the buyer sign off on receiving a copy of the well report?

Anyone have a sample/template for a document to record to give notice of elevated manganese in well water.  Or maybe one dealing with arsenic?  Island County is demanding one but wouldn't supply a form.
