[WSBARP] Disclaimer of Inheritance

Hello Listserve Attorneys:

Does someone have a simple form to use as a Disclaimer of Inheritance?  My
client does not want to endanger her DSHS benefits by receiving $40,000.
The funds can go instead to her adult son.
