[WSBARP] notice to forfeit earnest money

I have a case where the prospective buyer has verbally told the seller
that he has decided not to go through with the sale.  I represent seller.
Verbally propsective purchaser has acknowledged that he will lose his
earnest money.
 
I have seen and used a form for a notice to purchaser that he will lose
his earnest money after ____ date.  I can’t find what I have seen/used. I
didn’t see one in the LPO forms.
 
Does anyone have a form to share for this?
