The LPB forms on the WSBA web site may have the form you can use, or
perhaps modify for your purposes:

Practice-Officers/LPO-Forms

The “Partial Waiver of Lien” sounds like what you seek.

Subject: [WSBARP] release of lien rights by contractor

Do any of you have a simple release of lien rights for a contractor to
sign when they receive a draw from the builder for work performed to that
point?

Sincerely,

Tom
