<div class="x_gmail_default" style="font-family:arial,sans-serif">1) Probate was opened on this date __________</div>
<div class="x_gmail_default" style="font-family:arial,sans-serif">2) Notice to DOR and DSHS occurred on this date ___________</div>
<div class="x_gmail_default" style="font-family:arial,sans-serif">3) There has been no communication from either as of today's date. </div>
<div class="x_gmail_default" style="font-family:arial,sans-serif">4) Based upon the information provided by the Executor/Administrator/PR - the estate is valued at less than the estate tax exemption threshold. </div>
