I’m not sure there is a definitive answer, but I tend to simply use the original trust name (date) and include “as amended”.

Subject: [WSBAPT] Trust restatement question

I restated a 2008 revocable living trust for some clients. Do I need to reference the restatement date in a quitclaim I'm doing, or just the original trust name and date?

The trust could be restated again, so I'm not sure it makes sense to reference the restatement date.
