[WSBAPT] Therapy Practice client record transfer

Hello everyone:

I am looking for a template for a client with an active therapy practice. My Client would like to designate a colleague who could accept client records for the purpose of client notification and transfer of records to a new provider(s) in the event of my Client’s disability or death.  I am looking for a HIPAA compliant agreement my client  would enter into with this colleague. HHS.gov has a template which I could modify, but if any of you has a good template for this purpose, I would appreciate it. Any other tips also appreciated.
