[WSBAPT] Guardianship: unknown person opened a bank account without authorization

An adult disabled person is being disqualified from services by ADD due to a Banner bank account in the disabled person's name. However, Guardian/Conservator never authorized this account, and Banner Bank will not provide any information. Saturday, I am leaving town—what can I do? Does anyone suggest a Show Cause? Would Banner Bank respond with information or say account contact info is private? I would appreciate it if anyone could share pleadings I could file before I leave. Thanks Mike
