Jeff,

I do not see that anyone has responded to your post.  In Family Law matters, it is common to consolidate DV actions with Family Law actions (divorce, parenting plan, etc.) 
I have also had this done years ago where one party filed a paternity action and the other filed a Third Party Custody (which no longer exists.) 

I drew a blank trying to think of a case so that I could send you a pleading. However, you just include the usual language of orders i.e. "motion hearing on xxx, on motion of xxx, in xxx case number to consolidate case X, case Y, and case Z."   (If court did it on it's own motion, you would say that instead.) 

If it was NOT an obvious consolidation (like DV and divorce) and there were findings, you could put those in there.   
Decide WHICH CASE WILL BE THE PRIMARY CASE - X for example- and end by saying that "The Court now orders case  Y, and Z to be consolidated into case X."  

Use the caption for the primary case, but beneath the case #,  put in parenthesis (with consolidated cases  X cause#, Z cause #).

You will do that for every single pleading your ever file in those cases going forward.

After that, when you look at the court docket, the NON PRIMARY cases will end.  Only the primary case will continue to have pleadings added.
