[WSBAPT] TEDRA Agreement to appoint non-probate notice agent

Listmates,
RCW 11.96A.220 requires a TEDRA agreement to appoint a notice agent for a
non-probate notice to creditors where there are multiple successors. I've
only ever done a non-probate notice whether there was a single heir. Before
I reinvent the wheel, does anyone have a good template for this agreement
they would be willing to share?
